Supplemental Forms for NEW Students
United Christian Education Center is licensed by the State of California Department of Social Services and must have the following forms on file for each child attending preschool.
NOTE: Please fill out all the forms, save to your computer and attach them in an email to school office, ucecenter@gmail.com for ease and convenience.
